Before and After-School Enrichment Program
Please click on the form(s) below.  Complete the registration form(s) in its entirety, and return to Coleman's main office with your pre-payment for the program.  Checks should be made out to Coleman Middle School.
If you are registering for both programs, a separate form must be filled out for each program.  Each form may only be used to register one student (please do not put siblings on the same form).
